Duties

- Monitor and document vital signs to assess patient health and respond to any changes in condition.
- Provide home community care, assisting clients with daily living activities tailored to their individual needs.
- Prepare nutritious meals and assist with meal preparation to promote health and well-being.
- Conduct regular patient observation to ensure safety and comfort, reporting any concerns to appropriate healthcare professionals.
- Support individuals with developmental disabilities, fostering independence and enhancing their quality of life through direct support.
- Engage in long-term care practices, developing meaningful relationships with clients while providing consistent support.
Requirements
- Proven experience as a nurse, caregiver, or in a similar role is preferred.
- Knowledge of hospice care practices and principles is a plus.
- Familiarity with monitoring vital signs and basic medical procedures.
- Experience in providing home community care services is highly desirable.
- Ability to prepare healthy meals and understand dietary restrictions.
