What are the responsibilities and job description for the RN Care Coordinator position at On With Life?

This position primarily works Monday-Friday 8am to 4 : 30pm, approximately 40 hours per week. This position also participates in a leadership on-call rotation, including shared weekday call, every 6th weekend, and every 6th holiday. IMPORTANT NOTE : Please be aware that our orientation process (typically up to 1 month) may require working days or shifts outside of what will be your permanent schedule.
What do Person Served Care Coordinators (PCC) do?
Under the direction of the Director of Nursing and in collaboration with other members of the rehabilitation team, the PCC is assigned a specific set of persons served for which the PCC has the following responsibilities :
- Collaborates with other rehabilitation team members, such as Nursing ,Therapy, Case Management, and Social Work to ensure comprehensive care of person served
- Collaborates with mid-level provider and medical director on changes in condition of person served
- Assists in education, monitoring and coaching the nursing staff on continuous quality improvement measures, and assists in implementing nursing policies and procedures
- Arranges transportation to follow up external doctor appointments, testing, etc.
- Communicates with person served family on continued changes or updates to plan of care
- Assists with discharge planning including family education and training
- Collaborates with other members of the nursing leadership team to provide support and coverage for other leaders as needed
