Low Caseload, Patient Focused : Join the OnCare Health Team
Our Home Health / Hospice CNAs Will : Work independently and take initiative to manage a caseload of about 14-15 patients.Have autonomy and flexibility in managing their schedule.Utilize varying skills as a member of the clinical team as well as the community relations team.Develop meaningful,effective, andprofessionalrelationships with communities, families, and patients.Provide patients assistance with personal care including, but not limited to, baths, skin care, back rubs, oral hygiene, hair care, dressing, meal assistance, and light housekeeping.Assist in supporting the patients psychosocial wellbeing through companionship.Assess and report on patients condition and significant changes to the RN Care Manager.Assist with community relations events and activities including Vitals Clinics, seasonal activities, education sessions, meet and greets, etc.
