Our Weekend Hospice Nurse Care Manager (RN or LPN) Will:
- Be available to work both weekend days.
- Provide individualized care by treating each patient as they would want a loved one treated.
- Develop care plans that reflect each person and their needs.
- Be detail-oriented, knowing that carefully managed patients (medications, treatments, communication, aide supervision, etc.) have more comfort and dignity.
- Focus on customer service when interacting with patients, families, communities/facilities, and all partners and co-workers.
- Help team members improve through instruction and support.
- Build and manage meaningful relationships with all partners.
Our Weekend Hospice Nurse Care Manager (RN or LPN) Has:
- An active state-required RN license or hold a compact state license.
- 2 years hospice experience.
- On-call/triage experience.
- An active Driver's License with a reliable vehicle, insurance, and an acceptable driving record.
- The ability to meet the physical requirements for patient care with reasonable accommodations (e.g., Independently lift at least 30lbs).
- Humility to know that even experts learn something new every single day.
- A positive attitude and desire to make every day better for the people they encounter.
