Job Description:

We are seeking a compassionate and skilled In-Home Therapist (LSW or LPC) to provide intensive home-based therapy to individuals and families in Cincinnati, OH. This role is ideal for a therapist passionate about making a direct impact by delivering mental health support in community settings.

As an In-Home Therapist, you will work with a caseload of 8-12 clients, collaborating with a care team to deliver evidence-based interventions aimed at stabilizing family dynamics and promoting healing.

Responsibilities:

• Conduct comprehensive assessments to identify strengths, challenges, and treatment goals.
• Provide intensive therapeutic interventions in clients’ homes and community settings.
• Implement evidence-based treatment approaches such as family therapy and trauma-informed care.
• Participate in an on-call crisis rotation, offering immediate telephonic and in-person support during psychiatric emergencies.
• Maintain accurate and timely clinical documentation, including treatment plans, progress notes, and discharge summaries.
• Collaborate with case managers, psychiatrists, and other care providers to ensure integrated client care.
• Ensure adherence to fidelity standards for home-based therapy, including flexible scheduling and crisis response.
• Participate in training and professional development opportunities to maintain licensure and enhance clinical skills.

Qualifications:

• Master’s degree in counseling, social work, or a related field.
• Active Ohio licensure as an LSW, LPC, LMFT (LISW, LPCC, or IMFT preferred).
• Experience: At least one year working with children, adolescents, and families in behavioral health, child welfare, or juvenile justice.
• Strong clinical assessment and treatment planning skills.
• Ability to engage diverse clients and families in a community-based setting.
• Reliable transportation and a valid driver’s license required.
• Flexible availability to accommodate client needs, including some evening and weekend hours.
