Overview:
Peer Bridger is needed to work with patients in all components of the CPEP. The Peer Bridger works individually with patients to increase their understanding of coping skills and wellness tools as well as available community resources that can support individual recovery. Ability to offer patients hope through connection and engagement on what is possible.
Responsibilities:
  • Offer hope through connection and engagement on what is possible
  • Support individuals in connecting or reconnecting to their community and relationships
  • Connect to and support individuals in various stages of navigation from CPEP to community, based on identified needs and requests for support
  • Provide validation and support through person-centered approaches and engagement
  • Advocate when requested by the individual
  • Assist in development of comfort and self-advocacy
  • Demonstrate personal self-help and wellness skills
  • Share known resources and knowledge on system navigation
  • Serve as mentor when appropriate on wellness and recovery
  • Assist in the exploration of personal goals that lead to life satisfaction
  • Engage in shared decision making with individuals to support voice and choice in all treatment decisions
  • Utilize evidence-based peer interventions such as Action Planning for Prevention and Recovery, Shared Decision Making, Psychiatric Advanced Directives, Building Recovery Capital and the Eight Dimensions of Wellness
  • Conduct informational groups for CPEP staff about the benefits of working with Peer Bridgers
  • Work collaboratively with the team to educate all involved on recovery, self-determination, shared decision making, dignity of risk and peer values
  • Supervising other Peer Bridgers if hired and compensated to do so
Competencies:
  • Maintain peer values in all aspects of the work
  • Understand and approach all individuals in a trauma-informed and responsive manner
  • Be able to build relationships with all members of the team
  • Willingness to learn new skills

Qualifications:

Education:

  • High School Diploma or GED
  • Certified in NYS as a Peer in one of the categories listed below.
Experience:
  • Needs to demonstrate experience working in a setting with Mentally Ill, SUD and other patients who present to an emergency setting for evaluation and treatment.

Knowledge and Skills:

  • Able to work with patients using peer skills, empathy, and knowledge of the mental health system
Certifications/Licensure:

One of the below certifications is preferred to have at time of hire, otherwise one of the below certifications must be successfully completed and obtained within one year of start date.
  • Hold current/active New York Peer Certification which could be one of the following:
    • New York Certified Peer Specialist, provisional (NYCPS-P)
    • New York Certified Peer Specialist (NYCPS)
    • New York Certified Recovery Peer Advocate (CRPA)
    • Credentialed Family Peer Advocate (FPA-C)
    • Credentialed Youth Peer Advocate (YPA-C)
Physical Requirements:
  • Position requires prolonged periods of standing, reaching, walking throughout the working day.
  • Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
