Patient Navigator
The Bilingual Patient Navigator links immigrant, refugee, and non-English speaking patients with health, psychosocial, and other services to using a culturally sensitive, patient centered approach. This position collaborates with internal and external partners to guide patients through the nuances of the US healthcare system.
ESSENTIAL FUNCTIONS
- Provide comprehensive health navigation including assistance with :
Health insurance enrollment and verification
Registration and appointment check-inPCP assignmentAppointment scheduling, confirmation, and remindersInterpretation and completion of forms and paperworkDirecting patients to internal departments based on their needsWorking with clinical teams to review and ensure understanding of treatment plansInterpreting messages between the care team and patientCoordinate with external immigrant and refugee services, creating partnerships to help identify and connect patients with OCH servicesDeliver patient education in the form of printed materials, group health workshops, telephonically, and verbally during visits to the health centerAssist with outreach telephone calls to reduce care gaps and keep patients engaged in careRemain current on health-related community resources and act as a point of contact, following up on resource needs and troubleshooting patient barriers to accessing services when necessaryAssist with translation of patient facing signage, documents, and educational material in collaboration with the communications departmentAssist with producing training for staff that promotes cultural awareness and sensitivityRepresent OCH at community events, providing information about services in multiple languagesAdditional Duties
Attend all scheduled staff meetingsWork collaboratively with all members of the care team and extended medical teamActive participation in team huddlesActive participation in quality improvement initiativesActive participation in multidisciplinary committees and workgroupsAssist with the training of new staff in collaboration with leadership and preceptors / mentorsCoverage of PSR and eligibility duties as needed based on staffing availabilityAll other duties as assignedMINIMUM REQUIREMENTS
High School Diploma or GEDAt least 2 years of customer service experienceAt least 1 year experience working with diverse populations including refugees and immigrantsFluency as well as the ability to read and write in Dari, Pashto, and EnglishUnderstanding and sensitivity to the cultural nuances and practices of Afghan communities, particularly regarding healthcare beliefs and practicesThe ability to problem solve independently as well as the ability to collaborate in a team setting to work toward a specific goalExcellent interpersonal and communication skillsCurrent BLS certificatePREFERRED BACKGROUND
Prior experience working in healthcare settings, preferably in a role involving patientinteraction or navigation
Existing knowledge of external, local resources and services for immigrants and refugeesDegree or certification in relevant field such has healthcare administration, interpretation, or linguisticsReasonable Accommodations
