Associate Social Worker (ASW) - Street Medicine
The Street Medicine ASW provides behavioral health services to individuals experiencing homelessness in their own environments, such as encampments, parks, and other non-traditional settings. The ASW will offer trauma-informed counseling, support, and referrals to address the mental health and social needs of this vulnerable population. Working alongside a multidisciplinary team, the ASW will help bridge gaps in care and reduce barriers by bringing services directly to those who may not access conventional healthcare systems.
This role is on-site in Midtown Sacramento, CA (95811).
Essential Functions
- Provide individual counseling using strength-based and trauma-informed approaches, meeting patients in non-traditional settings such as streets, shelters, and encampments.
- Conduct thorough behavioral health evaluations, developing personalized treatment plans and setting therapeutic goals in collaboration with patients.
- Offer ongoing support and follow-up to evaluate patient progress and modify treatment plans as needed, ensuring continuity of care.
- Administer therapeutic interventions including cognitive-behavioral therapy, behavior modification, and stress management techniques to address mental health and substance use issues.
- Collaborate with medical providers, nurses, case managers, and other members of the Street Medicine team to coordinate care and support the patient's overall health and well-being.
- Refer patients to additional mental health services, including psychiatric care and psychotropic medication management, when appropriate.
- Provide education and counseling on harm reduction, substance use, trauma recovery, and mental health strategies tailored to individuals experiencing homelessness.
- Participate in integrated systems of care, including staff meetings, case reviews, and supervision, to ensure a collaborative approach to patient care.
- Maintain detailed, systematic documentation of patient encounters and treatment plans in the electronic medical record (EMR), ensuring timely and accurate records for clinical and billing purposes.
