Job Summary:

Job Requirements:
Applicants must have a valid RN or LPN Licenses.
Job Duties:
Providing compassionate holistic care
Maintaining a safe, secure, and healthy work environment by abiding by One Cross policies and procedures and relevant legal regulations.
Maintain our positive culture and attitude.
Work well with patients and team mates in a fast-paced environment.
Meet with patients through telephone conversations who qualify for Chronic Care Management.
Assess and address barriers that prevent patients from managing their chronic conditions in their homes
Provide information about community resources and assist in connecting patients to them.
Create individualized care plans for patients to improve quality of care.
Assess patients' care gaps and provide plans to help patients fill these gaps.
Bridge the gap between the primary care provider and the patient by providing the provider with the medical needs of their patient.
Support and reinforce chronic disease management plan implementation in the patient's home.
Improve the quality of care as evidenced by improving outcome goals and statistics.
Maintains HIPPA compliant files and program records.
