Key Responsibilities:
- Provide exceptional customer service to patients and staff.
- Serve as a super-user for all population health platforms.
- Manage and coordinate strategic value-based care initiatives to improve quality performance, close care gaps, and reduce overall costs.
- Develop and revise frameworks to support high-quality care delivery across all patient populations.
- Collaborate with health plan leaders to address network adequacy and establish wrap-around services for identified gaps.
- Provide education, reporting, and work plans to help value-based provider groups achieve performance targets.
- Utilize Population Health tools and EHR systems to monitor and optimize resources for value-based care agreements.
- Evaluate patient care to ensure compliance with value-based program requirements.
- Create best practice workflows and process mapping to meet measure targets.
- Generate and analyze performance metrics, identifying areas for improvement.
- Maintain regulatory compliance for population health programs.
- Other duties as assigned.
Ideal Candidates Will Have:
- Licensed Registered Nurse preferred; Master’s level in Informatics a plus.
- Minimum 5 years of experience in a clinical or community healthcare setting.
- Familiarity with third-party payer regulations is preferred.
