TheOpportunity

One Medical’s HighRisk team is looking for a Field Medical Assistant (internallyknown as a Field Care Coordinator) to support our patients withcare outside of the traditional clinic setting - in their home andtelephonically after an acute inpatient event and as part ofroutine primary care services.

Reporting to the Manager,Tele-Assist Operations, Field Care Coordinators go into patients’homes to facilitate telehealth visits with a provider in order toreach more of our patients where they live.

This will includebuilding a trusted and supportive relationship with the patient andtheir family, performing clinical assessments, assessing homesafety, documenting medications and reviewing provider care planswith the patient.

The work may also include supporting andfacilitating access to community resources and service agenciesnecessary to promote patient health and well-being.

You will alsoassist in teaching patients and family members to use technology tocoordinate care with their care teams, as needed.

You are a compassionate, kind, and open-minded individualwho is comfortable engaging with patients and their caregivers orfamily inside their home.

You have excellent communication skillsand quickly build trust and rapport with your patients. You are anadvocate for patients, with a mastery of human connection and astrong drive for service.

You bring strong communication skills and enjoycollaborating with an integrated primary care team to facilitateoverall better healthcare outcomes for our patients.

You arecomfortable driving throughout Seattle to reach your patients intheir homes.

What you'llwork on :

• Facilitatetelehealth visits in a patient’s home, using technology to ensurethe provider has a complete view of the living situation and toensure the patient exam can be completed viavideo.
• Conduct standard patientassessments and documentation to facilitate a complete visit by thetreating provider in the home and to ensure patients understandtheir care plan.
• Clinical duties may includeproviding best in class telehealth set up services, performing andassisting with extended scope duties such as, but not limited toclinician guided physical exam, health status screenings, non-bloodspecimen collection, spirometry, retinopathy, EKGs,vitals
• Coordinate care with patients, familiesand other outside care teams (specialists, home health, etc.) asneeded. Connect patients to state and local community resourcesrelated to housing, transportation, food, and activities of dailyliving among other social and physical barriers tohealth.
• Assist patients and theirfamilies to fully participate in healthcare services, as well asnavigate and use community services or other resources in supportof their goals.

Assist patients with completion of applications foraccessing eligible benefits and resources

• Participate in the daily operations of the High Risk CareTeam, such as answering incoming phone calls, responding to emails,facilitating supports for transitions of care services
• Teach patients and family members to connect with theircare teams via technology

What you’llneed :

• At least 1 yearof experience working in home-based healthcare services or seniorliving care settings or Emergency Medical Technician (EMT)experience;
• Certified Medical Assistant(MA) and Basic Life Support (BLS) preferred or ability to obtaincertification within 6 months of hire required
• Strong written and verbal communication skills; Spanishspeaking is a plus
• A proven track record ofpersisting through change, demonstrating a forward thinkingperspective when under pressure, and consistently stepping up totake action on challenges
• A proven ability tobuild rapport and trust with patients or customers, whiledelivering individualized, human-centered, customer-focusedcare
• Clinical skills in performing basicvitals and clinical assessments preferred
• Experience facilitating access to community resources orsocial service agencies preferred
• Proficiencyin computer technology and video platforms such as typing,navigating the internet, meeting over zoom and using multiplesoftware systems simultaneously;

experience with Mac iOS, GoogleSuite preferred

Must have access to anindependent means of safe and reliable transportation as well asthe ability to travel house to house throughout the day;

This is afull-time role with shifts generally taking place between8 : 00am-5 : 00pm (Monday through Friday) based in Atlanta, Georgia.

Flexibility to travel throughoutthe day to patient homes in the Atlanta area and work in a varietyof Atlanta offices based on assignment.

Mileage will be reimbursedat the IRS / Federally established rate.

The base compensation range for thisrole is $21.25 to $22.25 per hour. However, actualcompensation packages are based on several factors that are uniqueto each candidate.

These factors include, but are not limited to,job related knowledge and skill set, depth of experience,certifications and / or degrees, and specific worklocation.

The totalcompensation package for certain roles may also include additionalcomponents such as a sign-on bonus, annual performance bonus,equity grants in the form of RSUs, medical and other benefitsand / or other applicable incentive compensation plans.
