Responsibilities
In this role, you'll provide best-in-class comprehensive primary care in an outpatient setting, partnering with our care team and leveraging our coaching and integrated behavioral health models to engage our geriatric patient population in care and drive behavior change. You'll utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population. You'll also have weekly protected time during the workday to gather with fellow One Medical Senior providers for team building and shared discussion of challenging cases.
Requirements
To be successful in this role, you'll need at least 2 years of primary care experience and Board Certification in Internal or Family Medicine. You should be able to thrive in a fast-growing, mission-driven organization focused on using data to improve patient outcomes.
