We are seeking a compassionate and experienced Director of Nursing (DON) to lead our home health team in Des Plaines, IL (60053). As the DON, you will be responsible for overseeing the clinical operations of the agency, ensuring the highest standards of patient care, and managing a team of skilled nursing professionals. The ideal candidate will have a strong background in home health nursing, leadership experience, and a passion for providing exceptional patient care.
Key Responsibilities :
Lead and manage the home health nursing team, ensuring compliance with federal, state, and local regulations.
Develop and implement patient care plans that promote optimal health outcomes.
Oversee clinical operations, including patient assessments, staff supervision, and quality control.
Ensure high standards of nursing practice, safety, and infection control.
Collaborate with physicians, therapists, and other healthcare professionals to provide comprehensive, patient-centered care.
Monitor patient progress and outcomes, making adjustments to care plans as necessary.
Maintain and update patient records in accordance with HIPAA and regulatory standards.
Assist with recruitment, training, and professional development of nursing staff.
Serve as a resource for clinical guidance and support for staff members.
Ensure compliance with Medicare / Medicaid guidelines and other payer requirements.
Qualifications :
Registered Nurse (RN) with an active, valid license in Illinois.
Minimum of 3 years of nursing experience in home health care, with at least 1 year in a leadership or supervisory role.
Current CPR certification.
Strong knowledge of home health regulations, Medicare / Medicaid requirements, and clinical best practices.
Excellent communication, organizational, and leadership skills.
Ability to work independently and manage multiple tasks in a fast-paced environment.
