One to One Health is looking for a Licensed Clinical Social Worker to join our Illinois team!

Position Summary: The Licensed Clinical Social Worker (LCSW) plays a pivotal role in supporting individuals, families, and communities by providing clinical assessments, diagnosis, treatment, and counseling services for a range of mental health and behavioral issues. The LCSW will work collaboratively with clients, families, and other healthcare professionals to develop and implement individualized treatment plans, while advocating for clients' needs and promoting their well-being.
Job requirements:
· Conduct thorough assessments of clients' needs, strengths, and challenges, utilizing evidence-based assessment tools and adhering to ethical standards.
· Develop and implement individualized treatment plans that address clients' specific needs, utilizing various therapeutic modalities and interventions.
· Provide individual, group, and family therapy to address a range of mental health and behavioral issues.
· Provide immediate support and intervention to clients experiencing crises or emergencies.
· Coordinate and manage clients' care, including connecting them with necessary resources and services.
· Maintain accurate and timely records of client interactions, assessments, and treatment plans.
· Collaborate with other healthcare professionals, families, and community agencies to ensure comprehensive and coordinated care.
· Advocate for clients' rights and needs, promoting social justice and equity.
· Stay current with evidence-based practices and ethical standards in clinical social work.
· Other duties as assigned
Qualifications
Skills/Knowledge:
· Knowledge of relevant laws, regulations, and ethical standards.
· Familiarity with local and regional resources for mental health and substance abuse treatment.
· Strong clinical assessment and diagnosis skills
· Proficiency in various therapeutic modalities and interventions. Excellent organizational and analytical skills
· Strong sense of professionalism.
· Proficiency in relevant software and technology systems.
· Ability to read, write, and communicate the English language.
· Ability to effectively communicate both orally and in writing
· Ability to follow written or verbal step-by-step instructions
· Ability to concentrate, think, and learn
· Ability to hear, see, sit, stoop, kneel, crouch, reach, lift, push and pull
Qualifications/ Experience
· Master of Social Work (MSW) degree from an accredited program
· Licensed Clinical Social Worker (LCSW) in the applicable state
· Minimum of 3 years of experience in clinical social work
