One to One Health is looking for a Part time or PRN Nurse Practitioner or Physician Assistant to join our Baldwin team!
Elected candidate will be eligible for a $5000 sign on bonus following 6 months of employment!

ESSENTIAL RESPONSIBILITIES:
- Provide family primary care services for employees onsite
- Manage acute and chronic health problems using evidence-based protocols
- Provide health screenings, immunizations, and other disease prevention programs
- Provide age-appropriate assessments and interventions in order to meet the needs of patients and/or families served
- Provide patient/family education, health coaching, and health advocacy services of patients and/or their families
- Provide occupational care, as needed
- Administer medicines and treatments
- Order, administer and analyze the diagnostic tests
- Monitor the effectiveness of interventions
- Collaborate with multidisciplinary team members by making appropriate referrals
- Enter patient data into the EMR system
- Assist in all areas of the clinic, if needed
- Perform related duties, as required
REQUIREMENTS
- Excellent verbal and communication skills
- Experience with EMR systems
- Adaptable in differing situations
- Able to multi-task and work independently.
- Must be flexible with hours/shifts, if assigned
- Demonstrate professionalism at all times
- Display teamwork with a team player mindset
- Be organized with attention to detail
MINIMUM QUALIFICATIONS:
- Certified as an Advanced Practice Nurse or Physician Assistant in the State of Wisconsin
- Have experience working with family care
- Licensed in the State of Wisconsin
- Prescriptive authorization
- Licensed as an RN and APN Nurse Practitioner (NP)
- Three (3) years of relevant clinical experience preferred

Qualifications
QUALIFICATIONS/EXPERIENCE
• Successful completion of an accredited NP or PA program according to Wisconsin State Law
• License and Certification as an Advanced Practice Nurse or Physician Assistant in the State of Wisconsin
• Prescriptive authorization
• Two years of relevant clinical experience
• BLS/ACLS
• DEA
• Certified DOT examiner preferred
