Now Hiring: Full-Time Outcome Coordinator
Location: Mason City, IA
Position Summary:
One Vision is currently seeking a dedicated Outcome Coordinator to join our team full-time. This role responsible for coordinating and monitoring Individual Support Plans (ISP) for up to 25 individuals. The Outcome Coordinator ensures that services and supports are effectively integrated to meet the unique needs, goals, and preferences of those we serve.
Key Responsibilities:

Coordinate, integrate, and monitor ISPs to ensure individualized support and progress toward personal goals.

Review and complete quality assurance checks for Home and Community-Based Services (HCBS) documentation.

Advocate for and promote the health, safety, and well-being of individuals served.

Ensure the rights of individuals are upheld with dignity and respect.

Maintain timely and accurate electronic documentation in compliance with One Vision policies.

Build and maintain professional, respectful relationships with individuals served, their guardians, funding sources, managed care providers (MCPs), case managers, and colleagues.
Required Skills & Abilities:

Strong verbal, written, and interpersonal communication skills.

Ability to appropriately respond to behaviors, crises, and emergencies; must be physically capable of implementing Mandt procedures per agency policy.

Ability to understand and follow written and verbal instructions, complete required documentation, and support individuals in activities of daily living.

Proficiency in Microsoft Office Suite (including Excel) and basic internet navigation.

Physical capacity to bend, kneel, stoop, and assist with transfers (up to 50 lbs), as needed.
Qualifications:
-
Bachelor's degree in a human services-related field, or RN license with at least one year of experience supporting individuals with disabilities.
- OR -
A minimum of five years of progressively responsible experience in the disability services field or equivalent. -
Must complete One Vision's Medication Administration course within six months of hire.

Employment is contingent upon successful completion of a background check and health screenings.
