One Vision is seeking to hire a Full Time - Outcome Coordinator (Office based in Clear Lake, servicing our Garner, IA Homes) - Our Outcome Coordinator is responsible for coordinating and integrating the Individual Support Plan (ISP) for up to 25 assigned persons served of One Vision (OV), monitoring the ISP and progress on goals/objectives and supports to ensure persons served needs are met.
Essential Job Functions:
- Coordinate, Integrate, and Monitor progress of the ISP for people served to ensure their needs are met.
- Monitor and complete quality assurance for HCBS intervention notes/services documentation.
- Promote and ensure the health, safety, and welfare of people served.
- Promote and ensure the human rights of people served are maintained and respected.
- Maintains and ensures required electronic documentation is completed in a timely manner per One Vision policy.
- Maintains and ensures appropriate healthy relationships and professional boundaries with people served, their guardians, Funders, MCP's, Case Managers and other entities, as well as co-workers.
Required Skills/Abilities:
- Exhibits excellent verbal, written, telephone, and interpersonal communication skills.
- Able to respond appropriately to behavior, crisis, emergencies, with the physical ability to implement Mandt procedures per One Vision policy.
- Comprehend and implement legible written and verbal instructions to complete required paperwork and support people served in completion of their activities of daily living.
- Exhibits moderate computer skills including internet and Microsoft programs, such as Excel spreadsheets.
- Physically be able to appropriately bend, stoop, kneel, reach with hands and arms, walk, stand, and balance self to support persons served in completion of their activities of daily living.
- Ability to lift and transfer per One Vision policy, up to 50 pounds.
Job requirements:
- BA/BS degree in a human service-related field or RN license with 1 year of experience in supporting people with disabilities. Or 5 years of progressively responsible and experience supporting individuals with disabilities/ equivalent experience.
- Completion of OV Med Administration within 6 months of hire.
- Offer of employment is contingent upon pre-employment background check, and health screenings.
