DISTINGUISHING FEATURES OF THE CLASS: This position exists in the Public Health Department and
involves responsibility for making home visits, and providing support, education, and outreach in areas of
maternal and child health, nutrition, immunizations, chronic and communicable disease, and pregnancy and
prenatal care, accessing medical and non-medical services, childhood lead, and car seat safety. The work is
performed in accordance with established policies and procedures and under the direct supervision of the
Environmental Health Director. The incumbent will work in close cooperation with, and under the guidance and
general direction of, Public Health Sanitarian and/or other appropriate health personnel. The incumbent does
related work as required.
MINIMUM QUALIFICATIONS: Candidates must meet the minimum qualifications at time of application. Either:
(A) Possession of Bachelor's Degree in health education, public health, social work, psychology, human
services, or closely related field; OR
(B) Possession of Associate's Degree AND two (2) years of experience within a health and/or social work
setting or closely related field; OR
(C) Graduation from high school or possession of a high school equivalency diploma AND four (4) years of
experience within a health and/or social work setting or closely related field.
SPECIAL REQUIREMENTS:
1. Possession of a valid New York State driver’s license at time of appointment. License must remain valid
throughout appointment. Certain assignments made to employees in this class will require access to
transportation to meet field work requirements in a timely and efficient manner.
2. Successful completion of a Community Health Worker Introductory Training course(s) within fifty-two
(52) weeks of appointment.
NOTES:
1. Candidates MUST submit with application a copy of transcripts showing degree area and date degree
conferred, if qualifying under (A) or (B).
2. Degree(s) must have been awarded by a college or university accredited by a regional, national, or
specialized agency recognized as an accrediting agency by the U.S. Department of Education/U.S.
Secretary of Education. If the degree was awarded by an educational institution outside the United
States and its territories, the candidate must provide independent verification of equivalency. A list of
acceptable companies who provide this service can be found at

3. Verifiable part-time experience as described above will be pro-rated toward meeting full-time
requirements.
