Duties and Responsibilities:
- Works as the primary contact and representative of the facility with Hospital Case Managers, Social Workers, Physicians, Liaisons, potential residents, and their families, before and during the admission process.
- Exercises discretion and independent judgment in the facility's decision-making process of accepting or denying all referrals for admission.
- Facilitate and coordinate with the Director of Nursing relevant medical information regarding potential admissions from hospitals, family members, and responsible parties on all patient admissions.
- Assist the resident and/or families in making appropriate admission decisions for rehabilitation by providing pertinent information and tours as needed.
- Respond to all phone, email, and walk-in tours and record all inquiries about the facility.
- Attend daily department head meetings updating daily census and admissions.
- Gather admission information and complete monthly reports.
- Must project a positive attitude and promote the facility and its staff/residents positively.
- Excellent computer, communication, and organizational skills required.
- Attend social events and conferences as deemed necessary by the Administrator.
