Primary Responsibilities
Hourly Rate: $18.00 – $23.00 (Based on Experience & Type of Service)

Primary Responsibilities
- Assists individuals in learning and developing new skills that will enhance opportunities for independence and inclusion. Support may occur in the home of the individual, or out in the community.
- Assists with attaining independence goals as described in the individual’s ISP to progress with their independence as adults.
- Produces accurate documentation of the services delivered. Documents care provided and response of the consumer/family. Submits time sheets and all other documentation in a timely manner.
- Performs all duties in an ethical manner and in compliance with local, state and federal laws, Medicare/Medicaid regulations and established personnel policies and procedures, including confidentiality and HIPAA guidelines.
- This position requires the employee to work in a variety of environments (housing styles, cleaning and maintenance, heating, and cooling) during visits to participants in their place of residence.
- Driver’s License and Reliable Transportation (Required)
- High school diploma or equivalent.
- Minimum age requirement of 18 years.
- Bilingual (Preferred)
Hourly Rate: $18.00 – $23.00 (Based on Experience & Type of Service)
- Supported Living Services (SLS) Caregiver: $20.00 – $23.00
- Respite Caregiver: $18.00 – $20.00
