Primary Responsibilities:
- Assists individuals in learning and developing new skills that will enhance opportunities for independence and inclusion. Support may occur in the home of the individual, or out in the community.
- Assists with attaining independence goals as described in the individual’s Individual Support Plan (ISP) to progress with their independence as adults.
- Produces accurate documentation of the services delivered. Submits Notes, time sheets and Mileage in a timely manner.
- Performs all duties in an ethical manner and in compliance with local, state and federal laws, Medicare/Medicaid regulations and established personnel policies and procedures, including confidentiality and HIPAA guidelines.
Physical Demands:
- This position requires the employee to work in family's homes and out in the community.
- PA Driver’s License required
- Valid Registration
- Valid Car Insurance
- Must be able to pass Child abuse and Criminal background checks.
Knowledge and Experience:
- High school diploma or equivalent.
- Minimum age requirement of 18 years.
