As a Registered Nurse Case Manager at OneWell, you will play a crucial role in providing both direct and indirect patient care using your professional nursing skills and knowledge. This role involves overseeing a caseload and collaborating with a team of RNs and LPNs within designated territories.
Responsibilities
- Conduct thorough patient assessments to identify individual needs and develop tailored care plans.
- Coordinate discharge planning processes to ensure smooth transitions from hospital to home or other care settings.
- Collaborate with multidisciplinary teams to provide holistic care that addresses physical, emotional, and social needs.
- Educate patients and their families about health management strategies and available resources.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Advocate for patients' rights and access to necessary services within the healthcare system.
- Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.
- Participate in quality improvement initiatives to enhance service delivery within the organization
Qualifications
- Current Registered Nurse (RN) license in good standing.
- Proven experience in case management or related nursing roles, preferably within a healthcare setting focusing on developmental disabilities.
- Strong skills in discharge planning and patient assessment processes.
- Familiarity with primary care practices and interdisciplinary collaboration.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to advocate for patients' needs while promoting their independence and well-being.
- Experience working with diverse populations including geriatrics or pediatrics is a plus.
