We are looking for a mission driven Social Work Community Health Navigator to join our team!
The Social Work Community Health Navigator assesses the social determinants of health and other related needs of patients and connects them to appropriate resources within the community. The position requires a working knowledge of social determinants of health and how they impact the overall health and quality of life of patients. The SW Community Health Navigator serves as a liaison between external community partners and clinical staff members of One World to ensure patients have access to resources and to remove barriers to medical care.
This is a full-time position located on our main campus(30th& L). The schedule will be 40 hours per week; generally, Monday-Friday, 8am to 5pm.
- One year of experience working within health care or assisting individuals with social services.
- Excellent verbal and written communication skills.
- Proficiency in both English and Spanish required.
- Task and detail-oriented with excellent customer service and organizational skills.
- Great computer proficiency with Microsoft Office and a willingness to learn other programs.
- Working knowledge of the Latino culture and commitment to cultural competence.
- Familiarity with social service resources in the community.
- Willingness to complete home visits as deemed necessary and a valid drivers license.
- Prior experience in completing needs assessments and evaluations is preferred.
- Two years of human services or community networking experience in a healthcare or social services environment is preferred.
- Functions well in a team environment.
- Motivated and self-directed.
- Experience working with Domestic Violence, Child Protective Services (CPS) and Adult Protective Services (APS) is a plus.
- High school diploma or equivalent required.
