We are looking for a mission driven Obstetrical Case Manager to join our team!
The OB Case Manager works with pregnant patients determined to be at risk of having a negative maternal or infant health outcome. This person will collaborate with the clinic, patient, and outside resources beginning anytime in pregnancy and concluding at birth. The OBCM will be responsible for all documentation related to OB Case Management visits, assessments, care plants, etc.
This is a full-time position located on our main campus (30th & L). The schedule will be 40 hours per week; generally, Monday-Friday, 8:00am to 5:00pm.
