DESCRIPTION
The Onondaga County Health Department is seeking a qualified candidate to support the Mental Health and Substance Use Initiatives Program in Onondaga County. This position will work with community members with substance use disorders to achieve their individual goals and improve their access to community services and the healthcare systems.
The Case Manager (Health) will work collaboratively within the community to develop relationships with clients, work to develop individualized plans for each client and foster relationships with other service providers. The Case Manager regularly engage in community outreach and will cooperate with the entire Substance Use Initiatives team to provide comprehensive, empathetic care. The position reports to the Program Coordinator of Substance Use Initiatives.
· Provide assistance in establishing in community care.
· Act as a referral agent for patients to other county and community-based programs and services, including substance use supports. As appropriate, refer participants to programs that offer long-term services such as benefits/entitlement programs, Peer services, and supportive housing.
· Coordinate and collaborate with care managers and patient navigators from community agencies including maintaining direct communication with collaborators and participant to facilitate coordination of care.
· Maintain contact with clients, including phone contacts and field visits.
· Facilitate transition to additional levels of services and supports with the participants.
· Conduct outreach efforts to participants who have been disengaged from services and/or individuals who are seeking / may benefit from assistance for issues related to substance use.
· Connect individuals and families to identified substance-use supports and services.
· Develop and/or complete individual service plan with participants.
· Assist participants to develop life skills in the direction of maximum autonomy.
· Visit hospitalized patients.
· Assist with crisis management.
· Perform other related duties as required.
· Complete required documentation, including tracking mechanisms, progress notes and care plans in accordance with agency standards.
· Attend and participate in monthly staff meetings.
· Participate in weekly individual supervision.
· Participate in staff training and program development meetings.
· Participates in public health preparedness activities as trained and assigned.
· Utilizes appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial and ethnic backgrounds, sexual orientations, lifestyles and physical abilities.
When Assigned to Transition Support:
· Assist individuals with substance use disorder to successfully transition from hospital/rehab/jail to the community.
· Work to identify and/or develop an individual’s wrap-around support to collaboratively develop, implement, and evaluate comprehensive transitional care interventions for high-risk clients.
· Follow up with participants 30 days post discharge.
· Escort participants to appointments, as needed.
MINIMUM QUALIFICATIONS
A. Graduation from a regionally accredited college or university or one accredited by the New York State Board of Regents to grant degrees with a Bachelor’s degree in any field and two (2) years of experience, or its part-time equivalent, as a professional advocate or service provider in the field of rehabilitation, substance use disorder recovery, substance use disorder treatment or mental health; or,
B. Graduation from a regionally accredited college or university or one accredited by the New York State Board of Regents to grant degrees with an Associate’s degree in any field and four (4) years of experience, or its part-time equivalent, as a professional advocate or service provider in the field of rehabilitation, substance use disorder recovery, substance use disorder treatment or mental health; or,
C. Graduation with a high school diploma or holder of a high school equivalency diploma issued by an education department of any of the states of the United States or holders of a comparable diploma issued by any territory, commonwealth, or possession of the United States or by the Canal Zone or holders of a report from the United States Armed Forces certifying successful completion of the tests related to general education development, high school level and six (6) years of experience, or its part-time equivalent, as a professional advocate or service provider in the field of rehabilitation, substance use disorder recovery, substance use disorder treatment or mental health; or,
Completion of a Master’s degree in health education, public health, health promotion, nursing, public health administration, public administration or a closely related health field may be substituted for one (1) year of work experience.
Desired Experience:
· Working knowledge of the principals and practices of substance use disorder treatment and prevention.
· Working knowledge of local agencies and Health Department services.
· Ability to assist individuals and families in navigating the Substance Use Services system.
· Ability to assess needs and then develop a person-centered plan with the individual.
· Ability to establish effective working relationships with health professionals, program participants and clients
· Ability to follow oral and written instructions.
· Ability to understand and interpret written information.
· Ability to communicate information to program participants.
Other Requirements:
· Must have a valid New York State Driver’s License.
· Be able to sit or stand as needed, with or without reasonable accommodation.
· Be able to walk, primarily on a level service, for sometimes extended periods throughout the day, with or without reasonable accommodation.

PROVISIONAL APPOINTMENT
Successful candidate will be appointed provisionally and must take the corresponding Civil Service exam, scheduled for a later date, and place high enough on the list to qualify for permanent appointment.
Position Schedule: 35 hours per week, Monday – Friday, 8:30am – 4:30pm. Candidates must have the ability to work occasional flexible hours, which may include evenings and weekends.
