Public Health Social Work Assistant – Full Time Permanent
Location: 421 Montgomery Street, Syracuse, NY 13202
Salary: $56,789 per year
The Onondaga County Health Department Lead Poisoning Prevention Program is seeking a qualified candidate to join our case management team.
Typical Work Activities:
Under the supervision of the Program Coordinator, the Public Health Social Work Assistant will provide comprehensive care coordination services to the families of children with elevated blood lead levels. This position is responsible for assessment and management of the needs of families with social and health care needs related to childhood lead poisoning. Care coordination may include but is not limited to: activities necessary to ensure timely follow-up testing, parent education and support, effective communication with health care providers, linkage to community services, and if necessary, assisting families in need of relocation with accessing funding and services to secure safer housing.
This position will be required to coordinate with other county services including home lead inspectors, public health nursing teams, early intervention and pre-school programs, as well as external partners and service providers. This position will be required to complete required documentation of services provided. Participation in program, division and department meetings, quality assurance/improvement and performance management activities are required.
Minimum Qualifications:
Graduation from a regionally accredited college or university or one accredited by the New York State Board of Regents to grant degrees of a Bachelor’s degree or higher in sociology, social work, psychology, special education, early childhood education, human services* or a closely related field; or,
Four (4) years of professional or paraprofessional work experience in social work, social casework, human services* or early childhood or special education.
NOTE: Human Services comprise those career fields in which practitioners help others to solve their physical or emotional problems or to meet their physical, emotional or mental needs, or to enhance their cultural, social, spiritual or physical conditions. Practitioners in this class are typically characterized by working with people on an individual or group basis. Responsibility for the custody or detention of individuals or for determining program eligibility is not considered qualifying experience.
Desired Experience:
- Good knowledge and experience in the principles and techniques of social casework, interviewing and assessment, documentation and reporting practices, and confidentiality.
- Working knowledge of the social, medical, public health, educational and human service agencies and resources available within the community.
- Ability to interact effectively and professionally with persons of diverse cultural, socioeconomic, educational, racial and ethnic backgrounds, sexual orientations, lifestyle, and physical abilities.
- Valid NYS Driver’s License
Position Schedule: 35 hours per week, Monday – Friday, 8:30am-4:30pm.
