Performs intermediate professional work coordinating a program or unit and providing direct human services and counseling services, and related work as apparent or assigned. Work is performed under the general direction of the Social Work Supervisor. Continuous supervision is exercised over care management staff.

Care Management for High-Risk Pregnancies is outcome-focused, with an emphasis on improving birth outcomes through reducing the rate of preterm and low birthweight births and monitors the pregnant Medicaid beneficiary population and prenatal service delivery system using data. CMHRP applies systems and information to improve care and assists members in becoming engaged in a collaborative process designed to manage medical, social and behavioral health conditions more effectively.
The CMHRP Program provides a variety of services in the form of population management and direct care management, as integral members of the maternity care team. Once identified and actively receiving CMHRP Services, members must have clear documentation of a Comprehensive Needs Assessment-OB, interactions, care plan and goal(s) and other Care Management for High-Risk Pregnancies activities. Specifically, CMHRP activities done with or on behalf of members are recorded as interactions, including outreach to members to engage them in care management.
Position Responsibilities
Domain 1: Data Analytics and Assessment Skills
- Monitor quality and quantity of work through case staffing and records review.
- Access existing quantitative and qualitative data to determine data needs and evaluate the quality.
- Apply public health informatics in using data, information, and knowledge.
- Monitor the work of subordinate staff through case reviews, progress reports, conferences, and informal discussions to assure compliance with operational standards and established policies.
Domain 3: Communication Skills
- Establish and maintain relationships to improve community health and resilience.
- Collaborate with community members and organizations.
Domain 4: Health Equity
- Assess needs of client population as well as overall agency goals in order to develop program and plan work operations which involves setting priorities and establishing goals for the program.

- Advocate for health equity and social and environmental justice.
Domain 7: Management and Finance Skills
- Make recommendations on budgetary needs, space, and equipment.
- Monitor expenditures and staying within predetermined budgeted amounts and project annual budget for the program for consideration by the supervisor.
- Engage in contingency planning and performance management.
- Facilitate collaboration among individuals, groups, and organizations.
- Assign work and are responsible for maintaining workload balance within the program and developing procedures to improve efficiency.
- Provide orientation and training to staff and recommend additional training to supervisor.
- Meet with staff individually or in groups on a regularly scheduled basis to discuss workflow and policy or procedure changes.
- Counsel staff as necessary and may carry out limited disciplinary actions, recommend merit increases and promotions, and participate in the selection process of new employees.

Qualifications
Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group work and community problems. Knowledge of the psychosocial, socioeconomic, and behavioral problems and their treatment. Knowledge of governmental and private organizations and community resources. Knowledge of laws, regulations and policies which govern the program. General knowledge of medical terminology, disease processes and their treatment are required in certain programs or settings. Skill in establishing rapport with a client and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client's problem. Ability to establish and maintain effective working relationships with administrative superior members of caseload and their families, and with civics medical, social, and religious organizations. Ability to train or instruct lower-level social workers, students, or interns. Ability to express ideas clearly and concisely and to plan and execute work. Ability to operate a personal computer, standard office productivity software and proficiency with the technologies required to perform care management functions. A high level of professionalism and possess appropriate skills needed to work effectively with the target population at high risk for adverse health outcomes. Motivational Interviewing skills and knowledge of adult teaching and learning principles. Ability to effectively communicate with families and providers. Critical thinking skills, clinical judgment, and problem-solving abilities.
Education, Experience, and Special Requirements
Human Service Coordinators must have at least one of the following qualifications:
i. Social workers with a Bachelor of Social Work (BSW, BA in SW, or BS in SW) or Master of Social Work (MSW, MA in SW, or MS in SW) from a Council on Social Work Education (CSWE) accredited social work degree program.
ii. Registered nurses
iii. Bachelor’s degree in a human service field with 5 or more years of care management/case management experience working with the specific population of low-income, pregnant individuals and/or children ages 0 to 5 years
iv. Bachelor’s degree in a human service field with 3 or years of care management/case management experience working with the specific population (low income, pregnant individuals and/or children ages 0 to 5 years and has certification as a Case Manager (CCM preferred)
Supervisors providing services to the target population hired prior to Sept. 1, 2011 without a Bachelors or Master’s degree in Social Work may retain their existing position only. This grandfathered status does not transfer to any other position.
Supervisors who carry a caseload must also meet the CMHRP care management competencies and staffing qualifications.
