What are the responsibilities and job description for the Outreach Peer Support Specialist position at OnTrack Rogue Valley?
JOB SUMMARY: Peer Support Specialist (PSS) is an individual with sufficient recovery to provide peer support to individuals that are receiving services from OnTrack. Peer Support Specialists will assist the client in identifying and addressing possible barriers to long-term recovery. Maintains good working relationships with community partners and connects clients to additional resources needed to support recovery. The Peer Support Specialist is an active member of the Treatment Team and will utilize their own recovery experience to support and advocate for their peers. PSS will support the client by being active participants in their own recovery.
Essential Functions:
· Establish rapport by demonstrating and maintaining clear, consistent, appropriate, and therapeutic boundaries.
· Ability to work independently, professionally, and collaboratively with clients and other agencies’ staff.
· Concisely and thoroughly document daily clinical information.
· Demonstrate knowledge and experience applying principles of Peer Support Specialists and the Recovery Model.
· Assists clients in identifying their recovery goals and strategies to monitor progress.
· Assist client in identifying and utilizing natural supports.
· Understands the value of self-care and can teach and model this for the clients.
· Identify and maintain current information concerning community resources (housing, food, banks, heat assistance, etc.) and application procedures for accessing public assistance resources (TANF, 01IP, HUD, Head-Start, etc.).
· Assist individuals in meeting necessary requirements to retain services and supports.
· Support Client in gaining skills to live a self-directed life.
· Collaborate with staff and participate in treatment teams.
Education/Licensure/Certification requirement:
· Current, valid, Peer Support Specialist or Certified Recovery Mentor certification
· Register and maintain good standing as a Traditional Health Care Worker through the Oregon State Office of Equity and Inclusion.
· High School diploma or equivalent
· Current valid driver's license, and insurable under company policy
