About the position
This particular position provides in-home supports for one client in their home for 8 hours per day, Mon-Fri, with meal breaks. Scope of service is highly focused on home management and organization. This client needs a lot of support with organizing, purging and establishing systems to maintain an orderly home environment. Also, includes a variety of other supportive activities such as transporting client to appointments, completing paperwork, making calls and light housekeeping.The ideal candidate must be able to reliably commute to St Paul and be flexible to adapt the schedule occasionally to suit client needs. Hours are 9:30am-6:00pm. Also, client has one cat so candidate must be cat-friendly.

Duties
Job Title: Direct Support Professional

General Job Summary
This position provides Direct Support for people with disabilities living independently in the community. This includes the elderly and others with special health care needs. Direct Support Services are provided in the recipient’s home or in the community when normal life activities take him/her outside the home. Direct Support Professionals are responsible for providing direct support, positive direction, and assistance to individuals in accordance with program policies and procedures, and personal outcome measures.
Essential Duties and Responsibilities
A. Administrative
Timely and accurate completion of:
- Electronic timesheets and daily client activity logs.
- Other documentation/paperwork as required.
- Adhere to the Vulnerable Adult and Child Abuse and Neglect Policies and Regulations, the Health Insurance Portability and Accountability Act (HIPAA) privacy policies and maintain confidentiality of all information pertaining to clients, families, and employees.
- Maintain positive working relationships with designated Manager, Human Resources, Waiver recipients as well as their Responsible Party and /or family members.
- Independently manage own time in order to ensure assigned tasks are completed each shift.
- Participate in meetings/care conferences, as needed.
B. Services and Procedures
- Provide Support to people by assisting in the development and implementation of all support plans, including spiritual life supports, assisting people in achieving their personal goals and desires, providing interaction and choices for activities that support a meaningful day, and supporting people to be independent in activities of daily living.
- Assist with information gathering for personal outcome measures, and other assessments.
- Assist people with maintaining a clean, safe, and orderly home by following safety and infection control procedures, educate people about maintaining homes.
- Maintain and expand skills and knowledge relevant to providing high quality support and services. Obtain and maintain all required certifications and training and applies the information to support people served.
- Ensure that people exercise their rights and that rights are not restricted without due process, ensures people are treated with dignity and respect.
- Maintain open communication and cooperative work effort with Lead and other staff to assure quality and continuity of support to people served.
Perform other duties as assigned.
C. Qualifications/Requirements- Education and Experience
- Eighteen (18) years of age or older.
Applicant must successfully complete one or more of the following:
- Prior completion of training program that provides the individual with the skills required to perform personal care assistance/waiver services.
- Completion of DHS positive supports training and orientation on the needs of the client within the first seven (60) days after client services begin.
- A nursing assistant training program or equivalent for which competency as a nursing assistant is determined according to testing administered by the State Board of Vocational Technical Education,
- A homemaker/home health aide pre-service training program using a curriculum recommended by the Minnesota Department of Health,
- An accredited educational program for LPNs or RNs, or
- One (1) or more years of experience in a home care setting is preferred.
D. Certificates, Licensure and Registration
- United States Citizenship or evidence of a valid US Alien Work Permit.
- Valid driver’s license, with an acceptable motor vehicle review, and proof of auto insurance, if applicable.
- Position is subject to a satisfactory Minnesota Department of Human Services (DHS) background check.
E. Language Skills
- Must have excellent English speaking, writing and reading skills
- Excellent customer service and listening skills.
- Excellent oral and written communication skills.
- Ability to write clear, concise, and accurate correspondence.
F. Mathematical Skills
- Ability to do basic mathematics.
G. Reasoning Ability

H. Physical and Environmental Demands
· The physical demands described here are representative of those that must be met by employees to successfully perform the essential job functions of this position. In compliance with the Americans with Disabilities Act, reasonable accommodations will be considered.
· Moderate use of hands, wrists and fingers and lifting or moving up to 75lbs may be required.
· Movements such as stooping, crouching, bending, kneeling, climbing, and reaching are occasionally required.
· This position requires spending a majority of the workday standing and walking, with occasional sitting.
· Noise and/or level of distractions in the work environment is moderate.
· Specific vision abilities required by this position include close vision, distance vision and peripheral vision.
· Ability to operate a motorized vehicle and have reliable transportation for work in the community.
