Provide routine nursing services and care to the clients, observe client health status and report health concerns to the RN Supervisor, Health Services Director and Medical Director. Provide client health care education to other LPN's and Direct Support Professionals. Functions under the supervision and guidance of the RN Supervisor and the Health Services Director.
ESSENTIAL FUNCTIONS
Treat clients with dignity, in a professional and considerate manner.Implement effective treatment plans with functional health care treatment goals.Provide clients and Direct Support Professionals education on the health care needs of clients.Monitor clients for potential side effects of medications or treatments and report potential side effects to the Medical Director.Case manage the client health care for their assigned clients.Meet regularly with the Program Manager to discuss current health care needs of their assigned caseload.Ensure monthly summaries, quarterly and annual assessments are completed in a timely manner.Ensure the lab work of their assigned clients is obtained and reviewed by the Medical Director in a timely manner.Ensure documentation from all medical appointments is obtained and filed for their assigned clients.Ensure all vitals are obtained following established protocol or healthcare provider order.Monitor the weights of the clients on their caseload and report any concerns with client weight to the RN Supervisor or Health Services Director.Maintain medical supply inventory for their assigned caseload.Round with the clients from their assigned caseload daily and assist with medication and treatment delivery.Assist their assigned clients with seeing the Medical Director and advocate for the client needs.Assist with on-call coverage and work with the RN on-call to meet clinical and staffing needs.Observe, evaluate and document the results of medications and treatment interventions.Ensure CMS and DIDD health care regulations are met for assigned clients.Assist clients with activities of daily living when the client is unable.Communicate health care needs to client families and provide comfort when appropriate.QUALIFICATIONS
Education & Experience :
Graduate of an accredited LPN school.Current LPN Licensure from the TN Board of Nursing.Current CPR certificationA minimum of five years (5) of LPN experience preferredExperience working with individuals with developmental disabilities preferredBasic knowledge of state, federal and ICF / IID laws and regulationsMust have valid driver's licenseAttend required Crisis Prevention Intervention (CPI), Abuse & Neglect, and Americans Disability Act (ADA) trainingCompetencies :
Good oral, written and interpersonal skills to effectively interact with clients, staff and managementCommitment to providing excellent service to clients; ability to remain calm and courteous when handling difficult requestAbility to maintain confidentiality regarding all documents and communicationsAbility to interpret and follow oral documented procedures, instructions, laws and policiesAbility to project a positive and professional image on behalf of the organizationMust be an effective team memberAbility to prioritize and manage time effectively.
