BOYS HOME CASE MANAGER

BACHELOR'S DEGREE REQUIRED

The Case Manager is responsible for providing comprehensive case management services to boys residing in the home. This includes assessment, planning, coordination, and monitoring of services to ensure the well-being, safety, and development of each resident.

Key Responsibilities:

• Conduct initial and ongoing assessments of residents to identify their needs, strengths, and goals.
• Develop individualized care plans in collaboration with residents, families (if applicable), and other team members.
• Coordinate and monitor services such as counseling, education, healthcare, and recreational activities.
• Maintain regular communication with residents, families, and external service providers.
• Advocate for residents' needs and ensure they have access to appropriate resources.
• Document case notes, progress reports, and other required records accurately and in a timely manner.
• Attend court hearings, team meetings, and other relevant events as needed.
• Provide crisis intervention and support during emergencies.
• Ensure compliance with all legal, ethical, and organizational standards.

Qualifications:

• Bachelor’s degree in Social Work, Psychology, or a related field (Master’s preferred).
• Experience in case management, preferably with youth or in a residential setting.
• Strong communication, organizational, and problem-solving skills.
• Knowledge of child welfare laws and trauma-informed care practices.
• Ability to work collaboratively with a multidisciplinary team.

Pay: $35,000.00 - $37,000.00 per year
