Open Arms Home Health Care – Des Moines is seeking a Nurse with home care experience to fill the position of full-time home care nurse/case manager. There is a position that is at its Ames office. Open Arms is an accredited and Medicare certified Home Health Care Agency with its home office in Des Moines, Iowa.

Job Description:
We are looking for a thoughtful and compassionate person who enjoys working with the elderly outside a clinical setting. The position requires good time management skills while providing each patient with the time and attention necessary to meet his or her needs.
The home care nurse will work in collaboration with the Supervising Nurse and Agency Administrator, providing oversight of the delivery of services provided to agency clients. Responsibilities include:
· Assessments/OASIS evaluations, recertifications and charting
· Generate plans of care and assign appropriate cares
· Performing evaluation tasks, including vital signs and medication review
· Supervise provision of personal cares by home health aide staff
· Coordinate with therapists, physicians, care coordinators and other health care professionals involved in the patients plan of care
· Educate patients and their families on proper home health strategies and procedures
The ability to multi-task and problem solve is essential. The successful candidate will possess strong clinical, communication, team-building and interpersonal skills.
Qualifications include:
· Active Iowa RN License.
· Strong clinical and assessment skills.
· Prior home care experience – preferred.
· Experience with OASIS and Medicare/Medicaid regulations for Home Care - preferred.
· Prior experience in QA, Performance Improvement or Nurse Education preferred.
· Strong computer skills and experience with EMR software.
· Excellent communication, organizational and mentoring skills.
