We Overview:
We are seeking a compassionate, dedicated Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP) to join our dynamic team at our Federally Qualified Health Center (FQHC). This full-time, dual-role position will allow you to provide comprehensive, patient-centered care across both mental health and primary care settings. You will initially work primarily at our Mankato location, with the opportunity to split your time between Mankato and Shakopee locations as the role develops. While the role will primarily be in-clinic, we offer telehealth flexibility to help accommodate patient needs and ensure access to care.

Job Responsibilities
- Provide psychiatric services, psychosocial and physical assessment of patients, treatment plans, and mange patient care.
- Work as member of an integrated team, while also providing medical providers with consults for mental health issues as needed and collaborating with medical providers as needed.
- Establish diagnoses, order, and interpret studies, perform, and monitor therapeutic procedures, assess and/or manage follow up care.
- Prescribe and regulate medications relative to the patient’s health care needs.
- Record and document health appraisal data, in a timely and accurate manner, necessary to maintain and/or coordinate required services.
- Communicate findings of mental healthcare assessments, diagnostic tests, etc. to patients, physicians, and other health care providers in a timely manner and/or as determined by clinic policy/procedure.
- Develop, provide, and systematically re-evaluate the plan of treatment for patients/patients’ families.
- Make appropriate referrals for patients when appropriate services.
- Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders.
- Maintain documentation of patient encounters via progress notes, assessment/diagnostic information, and treatment planning as outlined in the Department’s policy.
- Maintain any other documentation required for insurance companies and MNDHS.
- Work as member of the medical team, assist the Physicians/Nurse Practitioners, Medical Services Manager, and ODHC Administration in ensuring an efficient and effective model of practice and a positive philosophy/work environment.
- Obtain patient history and perform assessment by observation, interview and examination.
- Order, perform and interpret diagnostic studies.
- Perform preventative health assessments, screening, immunizations and patient care.
- Provide direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
- Perform clinical procedures approved by supervising provider or clinic-based protocol.
- Counsel and educate patients concerning preventative health, treatment options and community resources.
- Document in the electronic health record according to the Advanced Practice Registered Nurse scope or practice.
- Preserve the patient’s control over decision making, assess the patient’s commitment to the jointly determined and mutually acceptable plan of care, and foster patient’s personal responsibility for health.
- Conduct age appropriate and comprehensive or episodic health assessments of patients with real or potential, acute, or chronic problems with emphasis on wellness and patient education.
- Perform physical examinations and order required immunizations.
- Establish diagnoses, order, and interpret studies, perform, and monitor therapeutic procedures, assess and/or manage follow up care.
- Prescribe and regulate medications relative to the patient’s health care needs.
- Record and document health appraisal data, in a timely and accurate manner, necessary to maintain and/or coordinate required services.
- Communicate findings of healthcare assessments, diagnostic tests, etc. to patients, peers, and other health care providers in a timely manner and/or as determined by clinic policy/procedure.
- Share in the rotation of clinical hours for late days and in the on-call provider duties.
- Qualifications:
Active, unrestricted PMHNP and FNP licensure in the state of Minnesota. - DEA License Required.
- Board Certification in both Psychiatric Mental Health and Family Practice.
- Minimum of 2 years of experience in both mental health and primary care settings preferred, ideally within an FQHC or similar community-based healthcare setting.
- Experience working with underserved and diverse populations.
- Strong communication, organizational, and time-management skills.
- Ability to work independently and as part of a collaborative care team.
- Familiarity with telehealth platforms and technology.
