Job Title: High Needs Case Manager
Work Location: In-Person Tempe or Phoenix, Arizona
Division/Department: Specialty Programs
Reports to: High Needs Case Manager Team Lead
FLSA Classification: Non-Exempt
Status: Full-time
Position Summary
Provide high needs case management and facilitate
services for enrolled clients and families. HNCM (High Needs Case Management) will
work with children and families that require constant communication and
coordination of services with clients who present danger to self or others.
Case Manager will assist in completing referrals for inpatient services, out of
home packets, coordinating with Juvenile Probation and Department of Child
Safety. Coordination of services include substance use, placement, behavioral
health, and transition to adulthood.

Preferred Education And Experience
- Bachelor's degree in a related field with 2 years of experience in the behavioral health field preferred, or 60 hours of course work in behavioral health related classes with four years of experience working in the behavioral health field preferred.
