Certified Home Health Aide (cHHA)
Position Overview: To provide direct Client care as assigned by Open Systems Healthcare, Inc. (OSH) in the Client home and community. To provide quality, safe and effective home care services while adhering to the independent living philosophy of the Client. To administer the home care services that reflect OSH’s mission, vision, and values. Essential Job Functions: A Certified Home Health Aide is a person who carries out health care tasks as an extension of a registered professional nurse. A CHHA also provides assistance with personal hygiene, housekeeping and other related supportive tasks for a client with health care needs in his/her home.
Ensure quality and safe delivery of home care services that align with OSH, federal andstate policies procedures as well as the Occupational Safety and Health Administration andCDC recommendations.
- Participate in implementation of Client plan of care in accordance to OSH policy andprocedures.
- Assist Clients with Activities of Daily Living and Instrumental Activities of Daily Living according to their plan of care.
- Demonstrates the ability to communicate effectively with the client and his/her familymembers.
- Assists the RN supervisor to make client visits by ensuring presence of self and client atthe time planned.
- Demonstrates the ability to communicate effectively with other members of the healthcare team and staff of the agency.
- Consistently reports occurrences to the Nursing Supervisor.
- Consistently adheres to universal precautions, aseptic technique and infection controlguidelines.
- Consistently implements care in a manner that is maximally safe for the client, his/herfamily, and self.
- Consistently seeks, accepts and implements suggestions to improve performance.
- Demonstrates respect for the opinion of others.
- Consistently assumes and follows through on the responsibility for assignment.
- Demonstrates the ability to function effectively under stressful situations.
- Maintains confidentiality of client observations and records.
- Utilizes time effectively, maintaining a consistent level of productivity.
- Completes the continuing education requirements annually (12 hours).
- Consistently complies with standards for attendance, absence notification, andpunctuality.
- Consistently demonstrates professionalism through appearance, performance andcommunication.
- Assumes responsibility for reading and comprehending all posted notices,communications and policies and procedures related to CHHAs
- Demonstrates competencies to provide care to patients of all ages.
- Respects the rights, privacy and property of others at all times.
- Comply with Federal, State and Local Laws.
- Comply with all OSH policy and procedure requirements.
Education and Experience:
- High School diploma or GED equivalency, preferred.
Required Credentials:

A Certified Home Health Aide:
- completion of a training program approved by the New Jersey Board of Nursing required
- in good standing, holding a current, valid certificate as a Home Health Aide issuedby the New Jersey Board of Nursing
- completion of the Home care test

A Nursing Student
- with a written New Jersey Board of Nursing waiver in lieu of a certificate
- completion of the Home care test
- Evidence of a Tuberculosis screening that meets CDC guidelines
- Evidence of competency as per OSH and State requirements.
- Must have a reliable form of transportation.
- Passed OSH competency exam with a score of 80% or hire.

Work Environment and Physical Demands:
- The employee is occasionally exposed to a variety of conditions at Client sites.
- Ability to perform tasks involving physical activity, which may include heavy moving,bending, lifting, and extensive self-positioning. Continual or almost continual standingmay be required.
