Shape Futures : Housing Readiness Case Manager Opportunity!

Openwork is partnering with the HAY Center to hire a motivated and compassionate Housing Readiness Case Manager . This role focuses on preparing youth ( ages 17 to 24 ) for independent living by guiding them through the housing and leasing process, ensuring they are equipped to secure and maintain stable housing.

Key Responsibilities

• Conduct housing readiness assessments and provide personalized case management support.
• Assist youth with completing housing applications, navigating voucher processes, and attending housing briefings.
• Prepare and submit necessary documentation to local housing authorities for initial applications and recertifications.
• Provide support during the move-in process and ongoing case management for residents at the HAY Center Campus.
• Collaborate with internal programs, particularly the In-Care Program, and participate in wraparound meetings.
• Maintain detailed case files and complete required data entry in a timely manner.
• Engage in HAY Center and Housing Continuum events as needed.
