POSITION PURPOSE :
The Case Manager / Employment Specialist will play a key role in supporting justice-involved youth through the development, coordination, and monitoring of their individualized case plans within the Re-entry Program. This position is based out of Hertford, Gates, Chowan, and Bertie counties and will require close collaboration with community partners to ensure that program participants receive the support and services they need, including employment / training, educational services, and other supportive resources.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
- Promote the mission, vision, and values of OIC, Inc.
- Manage a caseload of participants, specifically justice-involved youth.
- Provide individual case management sessions at regular intervals, addressing clients' needs.
- Conduct initial screenings to assess required services and support levels.
- Develop customized case plans for each client, identifying barriers to successful re-entry and outlining necessary actions to overcome them.
- Utilize knowledge of the North Carolina Judicial System and / or community resources to support justice-involved youth.
- Document all client interactions and progress, including contact details, outcomes, and follow-up plans.
- Work closely with community partners to ensure participants are connected to appropriate services.
- Perform other duties as assigned.
