Under the direct supervision of the Program Manager – Intake and Assessment Program is responsible for the implementation and coordination of consumer entry into the system. Responsible for inquiries and assignments related to closed and inactivated cases. Under general direction, performs a wide variety of fact finding and facilitative services.
HOURLY RANGE:
$23.4503 - $32.9968 per hour

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Guided by the agency and program mission statement and core values, conduct initial consumer intake and provide support for Consumer Services Coordinators in the Intake and Assessment Program.
- Interview and screen all applicants for service completing necessary intake materials and completing initial intake. Counsel and refer individuals not eligible for services to appropriate community resource(s).
- Complete necessary paperwork, reports, etc. on a timely basis, including the necessary IT processing for all cases opened to the intake caseloads.
- May be required to be a backup person to schedule all appointments for consumer assessments. Work with and modify the agency’s intake scheduling system to maximize its efficiency and accommodate necessary changes. Notify the Program Manager of any potential scheduling problems as they arise.
- Coordinate vendor referrals.
- Process vendor reports and POS for services rendered.
- Write letters and memos, etc., related to intake duties. Process faxes via e-mails, screen and route to appropriate consumer records.
- Assist in the collection of information, prepare reports and perform surveys related to the intake process as requested.
- Process all Reactivations/Re-openings in their assigned county jurisdiction and assign to ongoing Consumer Service Coordinator (CSC).
- Facilitate eligibility review team by coordinating activities with regard to cases whose eligibility is in question assuring that CSC obtains the required information and that a timely eligibility determination is made.
- Responsible for inquiries regarding agency services and all activity on closed and inactive cases. Handle all phone calls and mail and is responsible for administrative work on inactive or closed cases.
- Meet and greet lobby walk-ins who are requesting intake and information. Handle all incoming phone calls related to new intakes. Delegate tasks and route incoming phone calls to most appropriate individuals.
- Maintain an adequate skill level in interpersonal and community relationships. Assure that the consumers’ rights and dignity are maintained in the provision of services.
- Perform as a member of the team, answering questions, sharing expertise and generally contributing to the harmony of the team. Perform different or additional work as assigned by the Program Manager – Intake and Assessment Program.
- Comply with the Agency’s Affirmative Action Program.

- Successfully complete all assignments arising out of the agency’s Performance Contract.

- Comply with and ensure program compliance with agency’s Personnel Policies and Procedures.
- Assure that consumers’ rights and dignity are maintained in the provision of services.

MINIMUM POSITION REQUIREMENTS:
- A high school diploma with five (5) years of related work experience during which time increasingly responsible duties were assumed.
- In lieu of work experience, a Bachelor’s degree in social work or a related field from an accredited school.
- May be required to be bilingual.
