Monday-Friday 8am-5pm
Assists and counsels clients and families with financial, social and emotional concerns. Hugh Chatham Health at Home (formerly Yadkin Valley Home Health) proudly serves Alexander, Alleghany, Ashe, Davie, Forsyth, Iredell, Stokes, Surry, Wilkes and Yadkin counties in NC.
Education and Formal Training: Masters degree in social work from an accredited school of social work.
Work Experience: Minimum of one year experience in social work, preferably in a home care agency, or a hospital, outpatient clinic, rehabilitation center or mental health program.
Knowledge, Skills, and Abilities Required: Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with ill people; uses tact, sensitivity, sound judgment, and a professional attitude when relating with patients, families, and co-workers at all times. Actively functions as a patient advocate. Must develop knowledge of current home care emergency policies and procedures, fire, safety, disaster, and infection control policies.
