Key Responsibilities

  • Develop and implement individual program plans (IPPs) that emphasize a person-centered approach tailored to the unique needs of each client.
  • Conduct monthly assessments to monitor client progress and ensure the effectiveness of services.
  • Maintain accurate and up-to-date client documentation, including emergency contacts and progress notes.
  • Act as a liaison among family members, case managers, and behavior specialists to ensure cohesive support for clients.
  • Participate in quarterly and annual case conferences with relevant team members to review client statuses and program effectiveness.
  • Attend program, agency, and organizational meetings to support team collaboration and knowledge sharing.
  • Review all plans of care, facilitate appropriate waiver reimbursements, and ensure accurate documentation for client services.

Qualifications

  • Bachelor's degree in a professional field related to social services or disabilities.
  • A minimum of 1 year of experience providing educational or habilitation services to individuals with disabilities.
