Key Responsibilities
- Develop and implement individual program plans (IPPs) that emphasize a person-centered approach tailored to the unique needs of each client.
- Conduct monthly assessments to monitor client progress and ensure the effectiveness of services.
- Maintain accurate and up-to-date client documentation, including emergency contacts and progress notes.
- Act as a liaison among family members, case managers, and behavior specialists to ensure cohesive support for clients.
- Participate in quarterly and annual case conferences with relevant team members to review client statuses and program effectiveness.
- Attend program, agency, and organizational meetings to support team collaboration and knowledge sharing.
- Review all plans of care, facilitate appropriate waiver reimbursements, and ensure accurate documentation for client services.
- Strong verbal and written communication capabilities.
- Exceptional time management skills with the ability to multitask effectively.
- Sound judgment and composure in high-pressure situations.
- Highly organized with a keen attention to detail.
- Ability to work autonomously as well as collaboratively within a team environment.
- Bachelor's degree in a professional field related to social services or disabilities.
- A minimum of 1 year of experience providing educational or habilitation services to individuals with disabilities.
