What are the responsibilities and job description for the Supported Living QIDP Manager position at Opportunity Enterprises Inc?
The QIDP Manager is responsible for program development for client facilitation of the team approach, and acting as the QIDP, to ensure compliance with federal, state, agency, and program regulations, policies and procedures. In addition, they will oversee all aspects of their caseload including supervision or staff and the site.
DUTIES/RESPONSIBILITIES
- Function as the QIDP and member of the Interdisciplinary Team ensuring compliance with all State and Federal regulations;
- Conduct monthly review of client progress including all areas in the client’s home;
- Assist the IDT to develop and tailor IPPs to match client needs and wants through a Person Centered Approach;
- Ensure staff have necessary records, tools, devices or other items necessary to provide formal and informal training for clients;
- Assist the IDT to develop content of the behavior support plans and ensure appropriate behavior training is in place to meet the needs of the clients;
- Develop and ensure implementation of appropriate “individual program plans”, IPPs;
- Participate in quarterly and annual case conferences for clients with other appropriate members of IDT;
- Be the liaison with family, case managers, and behavior specialists;
- Refer, schedule appointments, and/or arrange transportation for clients, as needed, to appropriate agency and/or community resources;
- Update and maintain client files (master and home) including emergency information;
- Plan, direct and/or attend appropriate program, agency and other organizational meetings and/or in-services;
- Know how to read and understand an NOA and request appropriate waiver reimbursement from the Case Manager for client services desired;
- Review all Plans of Care when received to ensure correct services are listed;
- Review Therap documentation (GER’s, T Logs, etc.) on a daily basis and provide appropriate follow up (BDDS reports, IDT notification, etc.);
- Attend appropriate In-Services in accordance with Medicaid regulations;
- Demonstrate appropriate professional role model for staff and clients;
- Demonstrate teamwork with staff and clients in the agency (across all departments);
- Ensure compliance of agency and departmental policy and procedures;
- Responsible to ensure we are meeting necessary agency/state/federal regulations and policy and procedures;
- Be available as scheduled or as needed to provide/receive emergency communication/assistance during regular business hours and non-traditional work times, including but not limited to supporting staff through a client’s behavior in the home and transporting clients to the urgent care of emergency room;
- Required to be on call on a rotating basis and carry a cell phone;
- Expected to be reachable by phone for emergencies;
- Conduct Quarterly house checks;
- Direct Support Professional responsibilities as needed;
- Ensure treatment sheets are up to date and in each home at all times;
- Supervises and trains House Leads and Direct Support Professionals on the following: implementation of financial materials/needs, medications, policies, procedures, and other job functions;
- Plans, assigns and directs work; appraises performance; rewards and counsels for employees, addresses complaints and resolves problems with support of Human Resources;
- Ensures compliance of agency and departmental policy and procedures;
- Ensures that Clients, House Leads and DSP’s are treated with dignity and respect;
- Ensure that House Leads order medications;
- Maintain acceptable work standards in all areas;
- Assume other responsibilities as requested by the director.

EDUCATION
- Bachelor’s degree in a professional category and at least one (1) year of work experience in the area of educational or habilitative services for developmentally disabled persons. If candidate does not possess a Bachelor’s degree but has suitable experience of 2-5 years they may also be considered.
- High school diploma/GED required
- Must be 18 years or older.
