Description of role:
Under the direction of the Program Supervisor, Direct Care Staff supports adults with developmental disabilities who may have a history of behavioral challenges to accomplish daily living activities at the person’s home, at the work site, and/or in the local community. The Direct Care Staff helps the individual served to be a known and valued member of his/her local community. Hiring Certified Nurses Assistants (CNA) and Home Health aide.
This position provides supervision, instruction, housekeeping, laundry, meal preparation, shopping, emergency intervention, and personal care for individuals with developmental disabilities. Hours vary within a 24-hour, 7-day a week schedule, with part-time (less than 32 hours per week) shifts.
We are looking for people who enjoy working with people with disabilities, are interested in teaching and encouraging healthy eating and a positive lifestyle. Candidate’s availability and flexibility to work when needed is a plus.
Tasks:
- Provide community supervision to ensure the health and safety of the individual with a developmental disability that may have a history of criminal involvement and/ or behavioral challenges served;
- Assist individual served with planning meals, purchasing and preparing foods;
- Assist individual served with personal care including dressing, undressing, hygiene, toilet actives and other activities of daily living (ADL’s)
- Assist individual served with housekeeping duties and marinating their homes;
- Assist individual served in developing adaptive skills and independent living skills;
- Prepare and maintain records of assistance rendered;
- Maintain client confidentiality. Follow reporting procedures regarding denial of rights and special incident reports;
- Communicate with other team members regarding any changes in regulations, policies and procedures and general information pertaining to the program;
- Assist individual served with the coordination of medical and mental health services;
- Monitor client progress towards goal outlined in the Individual Service Plan (ISP) and submit weekly data on goal tracking;
- Support community inclusion and choice;
- Create and use natural supports and support individuals in a person-centered manner;
- Teach community awareness to individuals served while they access their community via public transportation and travel training;
- Attend required training and staff meetings;
- Adhere to and all Opportunity Foundation, Inc. policies, procedures and program rules;
- Other duties as assigned;
Skills and requirements:
- Must pass a background check (Child Abuse Index, Criminal Record Clearance);
- High School Diploma (or equivalent) required previous work-related skill, knowledge or experience preferred;
- Must be 18 years of age;
- Possess a valid state driver’s license;
- First Aid/CPR certification required (within 2 weeks);
