II. DESCRIPTION
A. Purpose and Scope of Position
To assist, while on duty, in the overall supervision of the residence, as well as provide
all help necessary to those who live in the home.
B. Responsibilities
Participate in assigned ELP, implement and chart all data for each person. Provision of a safe, sanitary home environment. Provide safe medication administration. Provide assistance as needed in all areas of home and personal care. Participate in self development as requested. Assist manager in maintaining proper ratios. Assist in staff training and corrective actions. Other duties as assigned by supervisor within the scope of the position.
THERAP proficiencies, including but not limited to S-Comms; T-logs (new, view, search); GER’s (new, review); ISP data (enter, search, basic reporting); Health Tracking (new, search data, search appointments, entering and changing medications); Attendance (enter, approve).
Assist people to choose where and with whom they live.
Provide necessary supports for people to have intimate relationships in their lives.
Actively solicit people’s opinions about their services and supports.
Assist people to build personal support networks.
Ensure choose their daily routine based on their individual, identities, preferences and
desires.
Ensure people have time, space, and opportunity for privacy.
Modify or adapt living environments to meet people’s needs.
Assist people to participate in the life of the community, through interaction with other members of the community.
Assist people to perform different social roles.
Support people to have friendships.
Ensure people are safe.
Ensure people are free from abuse and neglect.
Assist people to understand/exercise their rights.
Ensure no individual right is informally restricted

Qualifications
III. REQUIREMENTS
A. Education and Training
High School or equivalent.
B. Technical
Good verbal and written communication skills.
C. Experience
6 months experience working with people who are disabled.
