Need: Behavioral Health APP (Must have 2 years recent Behavioral Health Child/Adolescent prescribing experience)
Location: 326 Nicholas Rd. Fitchburg, MA
Hours: M - F; 8a - 5p
EMR: TBD
Compenstion: $140k - $170k potential sign on DOE
Relocation: Yes, up to $5k

Exempt: Reports to BH Director & Psychiatrist
Under the general direction of the Behavioral Health Director and Director of Psychiatry, coordinates the comprehensive health care needs of the specific child patient population and provides appropriate direct patient care through assessment, diagnosis, and treatment of specific conditions. Participates in educational and research activities of the department.
Major responsibilities:
" Collaborates with other providers to provide comprehensive integrated primary care to CHC Family Medicine patients.
" Assesses patient's physical and psychosocial health/illness status through history taking and physical examination.
" Diagnoses, manages, and appropriately refers patients with acute, episodic, or chronic illness according to department protocols.
" Orders appropriate laboratory and diagnostic tests according to established guidelines.
" Facilitates and coordinates patient care referrals to specialists and outside agencies
" Develops and provides educational materials and/or programs to patients, families, and other health care professionals.
" May participate in clinical and/or nursing research.
" Actively participates in the departmental quality assurance program.
" Serves on appropriate department and/or hospital committees.
" Demonstrate understanding and commitment of the health center mission.
" Demonstrate understanding and commitment to the established CHC Values and Standards.
" Performs other related duties as required and as assigned.
Minimum Qualifications:
" Massachusetts Licensed MLP-NP.
" Certified in Massachusetts to practice in the expanded role as a Clinical Specialist in Psychiatry & MH Nursing.
" CPR/AED Certified or equivalent
" 1 yr. min. child prescribing work experience.
" Current DEA and Massachusetts Controlled Substance licensure for prescriptive authority
" Demonstrated mastery of a specialty area or other related field normally acquired by 3-5 years progressively responsible nursing experience.
Supervision Received: Psychiatrist
Accountable to and receives general direction from the Director of Mental Health for administrative aspects and the Director of Psychiatry for clinical aspects. Functions independently in accordance with CHC nurse practitioner protocols.
Supervision Exercised:
" Provides functional supervision to nursing staff involved with the care of specific patients according to CHC guidelines.
Hours: Full time 40 hours per week. This is an exempt position that requires days.
Environmental Working Conditions: Normal office environment of a busy Community Health Center Behavioral Health Department
Physical Requirements:
" None or very limited physical effort required.
" Work environment involves minimal exposure to physical risks, such as operating dangerous equipment or working with chemicals.
