Position Summary: Assists developmentally, physically and cognitively disabled adult individuals with their appointment needs by coordinating with leadership team and physicians.

Essential Duties and Responsibilities:
· Schedules appointments for exams, physicals, eye exams, dental exams, flu shots, walk-in/urgent care, chiropractor; any appointments as deemed necessary
o Maintains schedule of appointments for all residents
· Assures residents are following special instructions prior to the appointment, e.g., if medications are to/not be taken, resident needs to fast, etc.
· Transports residents to/from appointments
· Attends appointments with residents
o Communicates to medical provider symptoms, changes or needs of resident
· Results of the appointment, follow-up appointments, prescriptions, etc. are communicated to the manager of the client verbally and on Medical Results Form
· Contact MD offices/pharmacy with order requests, refills, needs for update on prescriptions
· Communicates Rx changes/new orders to staff
· Reviews medication administration records (MAR) on a monthly basis, reports discrepancies to appropriate staff or pharmacy. May contact medical provider if needed
· Assures necessary paperwork is completed and up-to-date with medical provider
o Assures paperwork, forms, etc. are given to manager or guardian to obtain authorizations/signatures
· May assist residents into and out of automobile or wheelchair, to lavatory, and up and down stairs
· Destroys unused medications with assistance of another staff member
· Adhere to the “Core Values” of the organization while performing essential job duties, functions, and representing Opportunity Inc.

· Performs other duties as assigned by leadership
Qualifications:
· High school diploma or general education degree (GED)
· Experience with medical terminology and physician interaction beneficial
· Must be able to pass an essential function evaluation
· The employee must frequently lift, support, and/or move up to 75 pounds and occasionally lift, support, and/or move up to 100 pounds
· Two years of related experience working in the human service field is preferred
· Requires effective skill of writing and speaking clearly and effectively
· Must be able to interact with medical professionals effectively and advocate for clients
· Requires the ability to plan, prioritize, and organize work effectively
· Requires the ability to work independently
· Requires valid WI driver’s license, at least 3 years driving experience and a good driving record
· Ability to travel when needed for training and resident needs
· Requires ability to sit or stand for extended periods of time
· Must be able to pass an essential function evaluation
· The employee must frequently lift, support, and/or move up to 75 pounds and occasionally lift, support, and/or move up to 100 pounds
· Must be able to push, pull, and maneuver 75 pounds on a regular basis
· The ability to bend, stoop, and aid clients in ambulation is required
· Must be caring and compassionate, and want to make a difference in the lives of our residents/participants
Certificates and Licenses:
· PCW certification
· Transportation Department Drivers Training completion
o Valid Wisconsin’s driver’s license with a good driving record is required
