Job Summary
As a Residential Program Supervisor of In-Home Services, you will support people with disabilities to successfully live in their own homes and participate in their communities. You will assist these individuals with increasing their independence and quality of life.
- Responsibilities:
- Utilize person-centered planning and service delivery methods to develop and implement plans.
- Facilitate regular communication with the individual and their Interdisciplinary Team (IDT) regarding supports and services' effectiveness.
- Provide written documentation on daily progress related to the individual's plans and paperwork, as well as progress reports.
- Document accidents or incidents and follow guidelines for internal and external notification.
- Facilitate intake and IDT meetings.
- Maintain data confidentiality and case files.
- Secure funding for services provided.
- Make referrals for additional services as requested or required.
- Provide necessary documentation and paperwork as requested.
- Promote hobbies and recreational activities.
- Assist with meal planning, grocery/personal shopping, and cooking.
- Schedule and attend medical appointments.
- Participate in team meetings and write care plans.
- Support and develop independent living skills.
- Provide education and opportunities to problem-solve with regards to safety skills, social interactions, and self-advocacy.
Requirements

- A personal vehicle and valid driver's license (mileage reimbursement provided).
Designated Coordinator Qualifications
- A baccalaureate degree in a field related to human services, education, or health, and one year of full-time work experience providing direct care services to persons with disabilities or aged 65 and older, or equivalent experience.
- An associate degree in a field related to human services, education, or health, and two years of full-time work experience providing direct care services to persons with disabilities or aged 65 and older, or equivalent experience.
- A diploma in a field related to human services, education, or health from an accredited postsecondary institution, and three years of full-time work experience providing direct care services to persons with disabilities or aged 65 and older, or equivalent experience.
- A minimum of 50 hours of education and training related to human services, education, or health, and four years of full-time work experience providing direct care services to persons with disabilities or aged 65 and older, or equivalent experience under the supervision of a qualified staff member.
