The Organization
Operation Samahan (OpSam Health) is a Federally Qualified Health Center (FQHC) that serves the San Diego County community. Founded in 1973 in a barber shop in Downtown San Diego by Filipino health professionals offering services to immigrants who did not have the finances, linguistic skills, or ability to navigate the health care system. Today, Operation Samahan has evolved into a diverse community health center with services available in National City, Mira Mesa, City Heights, and Rancho Penasquitos. We provide primary care, dental, behavioral health, family planning, integrative health and wellness, healthy eating, civic engagement, and other health-related services to the uninsured, underinsured, and underserved families of all income levels, regardless of their ability to pay.
JOB PURPOSE
The Nurse Practitioner delivers compassionate, comprehensive care to patients by diagnosing and managing a variety of health conditions. This includes performing health exams, ordering and reviewing tests, and creating treatment plans tailored to each patient. They also prescribe medications, provide education on managing health issues, and guide patients on lifestyle changes like diet and exercise. Working closely with other healthcare providers, the Nurse Practitioner ensures patients get the support they need. They keep accurate records, follow up on patient progress, and are committed to improving care and promoting wellness for the community.
ESSENTIAL DUTIES AND RESPONSBILITIES
- Perform comprehensive health assessments, including obtaining patient histories and conducting physical exams.
- Order, interpret, and follow up on diagnostic tests (lab work, imaging, etc.).
- Diagnose acute and chronic health conditions (e.g., diabetes, hypertension, asthma).
- Develop and implement treatment plans for patients, including prescribing medications, therapies, and lifestyle recommendations.
- Provide health education to patients and families on disease prevention, lifestyle changes, and managing chronic conditions.
- Promote wellness initiatives such as smoking cessation, nutrition, exercise, and stress management.
- Coordinate care with other healthcare providers, specialists, and community resources to ensure optimal patient outcomes.
- Follow-up on patient progress and adjust treatment plans as necessary.
- Accurately document patient encounters, medical histories, diagnoses, treatment plans, and follow-up care in the medical record.
- Prescribe medications and monitor patients for any adverse reactions or side effects.
- Adjust medication dosages based on patient response and evolving health status.
- Advocate for patients’ healthcare needs within the community and healthcare system.
- Provide emotional support and counseling as needed.
- Participate in quality improvement initiatives to enhance patient care and improve practice efficiency.
QUALIFICATIONS
Knowledge, Skills, and Abilities
Education : Master’s degree in nursing (MSN) or Doctor of Nursing Practice (DNP) with a focus in family medicine.
Licensure : Active Nurse Practitioner license, certification as a Family Nurse Practitioner (FNP) from an accredited certifying body (e.g., AANP or ANCC).
Experience : Previous clinical experience in family practice or primary care is preferred.
Bilingual : Vietnamese (REQUIRED)
