Job Summary:
Responsible for the overall oversight of the wounds and skin concerns within the facility. They will lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation. the WCC will manage and coordinate the wound care between the ancillary wound care extenders and physicians caring for the resident.

They will lead weekly wound team rounds and processes, including rounds, measurements, interdisciplinary care plan review, treatment recommendations, physician orders, and required documentation. the WCC will manage and coordinate the wound care between the ancillary wound care extenders and physicians caring for the resident.
Essential Job Duties & Responsibilities:
- Work with the ancillary wound care team to coordinate care for residents with skin concerns within the facility.
- Obtaining treatment orders as needed; as well as assisting with modifications to orders if needed.
- Assess new admissions as they come into the facility within the day (or Monday morning post weekend) to verify skin assessments are correct.
- Review all admissions.
- Review all resident charts weekly to ensure weekly skin checks (PCC Total Body Assessment) are completed timely as required.
- Review charts for residents being followed by the ancillary wound.
- Update wound care plans.
- Spot checks and weekly audits on dressing changes completed.
- Hold weekly skin and wound meetings with the IDT (WCC, DON, UMs, RD, WC Ancillary extender, and MD (PRN).
- Assess and perform treatment for residents with stage IIIs, stage IVs and IHA one additional day during the week with a PN to follow in the chart.
- Demonstrated knowledge of payor sources, referral sources and community resources.
- Excellent verbal and written communication skills.
- Excellent interpersonal and customer service skills.
- Review and audit charts and treatments to ensure compliance maintenance within the program.
- Meet weekly with clinical team and WCC to review wounds and skin/nutrition concerns with the team.
- RN or LPN
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
