Optalis Healthcare is seeking a Part Time Social Service Assistant for our Shorepointe Skilled Nursing center located in St Clair Shores, MI.
Position Summary: There is a special relationship our Social Service team develops with residents and coworkers in a post-acute and long-term care rehabilitation center. Our Part Time Social Service Assistant will address residents and family members' concerns & questions, schedule care conferences, filter calls, complete forms, and assist with sending referrals as needed. Candidates should have knowledge and/or previous experience with the geriatric (elder) population and State regulations as they apply to skilled nursing (SNF) and long-term care experience.

- High School Diploma, Social Service experience will be a plus.

- Scheduling care conferences for new admissions.
- Scheduling Long-Term care conferences.
- Issuing discharge notices including (NOMNC'S, ABN'S, DENC'S).
- Fax referrals, assist in filtering calls, and helping obtain consents for ancillary services.
Experience:
- Minimum One year experience in Skilled Nursing Facility or Long-Term Care Facility is preferred.
