Key Responsibilities
- Conduct comprehensive wellness exams and assessments in patient homes and virtually.
- Educate patients and families on preventive care and available health programs.
- Document patient histories, assessments, and care plans accurately.

- Prescribe medications, order diagnostic tests, and collaborate with physicians.
- Participate in integrated care team meetings to enhance patient outcomes.
